Self-Pay Screening: $95 Flat Fee
If you are paying directly, the flat fee is $95 for both scans, due at the time of scheduling. No insurance is billed. At your visit you will sign our Financial Policy. No referral is needed.
Using Insurance: What You Need to Bring
If you are using medical insurance, your provider must upload a referral through the patient portal or fax it to our office at least 48 hours before your scheduled visit. The OCT scan is billed to your insurance. The fundus photo is not covered under insurance bundling rules, so it is billed directly to you as a 40 dollar charge. At your visit you will sign our Financial Policy and an Advance Beneficiary Notice for the fundus photo charge.
